Reason for transport
To be transported from St Ives Public School to Ckids Afterschool (registered as Ganeinu - Gan Izzy) for education and care.
Regular Transport
This authorisation is for Thursday 21 November 2024 only at the conclusion of School (3:00pm).
Description of Pickup location
Children will be collected by an aducator of the serivce form the Yarrabung Road gate of St Ives Public School.
Description of Destination
Children will travel to CKids, located at 27 College Cr, St Ives. The vehichle will drop the children off in the car park. Children will then proceed directly to the service entrance.
Means of Transport
Vehicle operated by the service.
Period of time for which the child is to be transported
Children will be collected from school between 3:00-3:05pm and will enter the vehichle by 3:10. Travel time will take no longer than 5 minutes. Expected arrival at the service will be 3:15pm.

Seatbelts and Safety restraints
Seatbelts are required to be worn when children are transported. Booster seats will be used on any vehicle with a capacity of 12 or less seats for those children who require them by law.
Risk Assessment
A risk assessment has been prepared and is available for viewing at the service.
Written policies and procedures for transporting children are available at the service and are also linked on the booking form. Safe arrival of children procedure will be followed as outlined in the safe arrival policy and risk assessment.
